If you have, please let us know ASAP.

Please note that, in order to verify that the change of contact details advice is coming from yourself, you must submit this advice in writing.  This must be signed by yourself or your legal guardian.  The written advice must contained your name, date of birth, old address and old phone number, as well as your new address and new phone number.

The advice may be sent to us in mail, or faxed to us on (03) 98771722.

Alternatively, you can come to the clinic reception in person, and fill out a new contact details form.

It is of vital importance that we have your most up-to-date contact details, for safety reasons (eg. notification of medical results, and urgent health recalls).
